Club Diabetes (www.clubdiabetes.org )is an SME providing the lifestyle program, that is available online through an app. It was the result of a collaboration between general practitioner Jacqui van Kemenade, behavioural scientists and diabetes experts by experience. The aim of Club Diabetes is to reverse Diabetes type II. Right now we are able to do so in 80% of the cases, meaning that these patients do not have to take medication anymore.

The Leiden University Medical Center (LUMC) (https://www.universiteitleiden.nl ) is one of eight University Medical Centers in the Netherlands, and has three core tasks: patient care scientific research education The LUMC employs 7000 people. Patient care is largely focused on highly specialised care, the sort of care that cannot be provided anywhere else. The LUMC also offers a broad package of top-level clinical care, including organ transplants, cardiovascular interventions and all types of bone marrow transplants. The research conducted in the LUMC is both fundamental and patient- and care-oriented. A considerable portion of the research focuses on the translation from fundamental research to its use in patient care (from bench to bedside and vice versa). LUMC also has a strong technical research branch, focusing on the development and clinical translation novel image acquisition and analysis technologies. It has a state-of-the-art infrastructure for translational molecular imaging, operated by multi-disciplinary research teams.

Imec has two research organizations in The Netherlands. One in Eindhoven, Holst Centre, a
research and innovation partner. Within Holst Centre imec is collaborating with the Dutch
research organization TNO. Holst Centre was founded in 2005 and specializes in health
technologies, flexible and wireless electronics and photonics.
The other research organization is OnePlanet Research Center. OnePlanet is a multidisciplinary
partnership with Wageningen University & Research (WUR), Radboud University, Radboudumc
and imec. OnePlanet was founded in 2019 and is applying the latest chip and digital technologies
are applied to create a society in which everyone can live healthily and has access to healthy

sustainable food.
In the Dutch imec organization, 180 employees with tens of nationalities work across the three

Anticipating demographic trends combined with technological innovation, remote patient management bridges the access barrier by providing care to patients wherever they are and whenever they need it. smartQare offers unique digital biomarker measurements and medical-quality analysis for use in the hospital, in healthcare facilities, as well as to reach patients at home in rural areas and connect patients with specialists they would otherwise not have access to.
